Client Policy Guidelines
I. Appointment Policy
Identification – A valid, government-issued photo ID is required at every appointment.
Accuracy of Information – Clients are responsible for providing the correct ORI number, agency information, and any other required details.
Included Materials – Two FD-258 fingerprint cards are provided per service. Additional cards are available for an added fee.
II. Cancellation Policy
We understand that schedules can change. If you need to cancel or reschedule your appointment, please notify us as soon as possible. While no fees are currently charged for cancellations, we appreciate timely notice so we can accommodate other clients.
We reserve the right to implement cancellation fees in the future, and any updates will be communicated in advance.
III. Consent Policy
By proceeding with this service, you voluntarily agree to be fingerprinted using the ink-based method. Fingerprints will be recorded on FD-258 or agency-approved cards.
You are responsible for providing accurate agency and ORI information. Dynamic Specimens will prepare and return completed cards directly to you but is not responsible for agency acceptance.
If prints are rejected due to technician error, one complimentary reprint will be provided.
IV. Privacy Policy
Your privacy is important to us, We do not share your information except as required to complete your requested services. Dynamic Specimens maintains the confidentiality of all personal information. Fingerprinting records are not considered medical information under HIPAA, but are safeguarded as personal data under PII.
V. Payment Policy
We accept card payments only. Cash, checks, or other payment methods are not accepted under any circumstances.
An invoice will be issued for each appointment and must be paid in full at the time of service.
Appointments are not considered complete until payment is received.
Failure to provide payment may result in cancellation of services and denial of future appointments.
VI. Safety and Behavior Policy
Client is expected to always treat all staff with courtesy and respect. Verbal abuse, threats, or inappropriate behavior will not be tolerated.
Any form of harassment, discrimination, or threatening behavior will result in immediate termination of services and potential reporting to authorities.
We have the right to refuse or discontinue services if client or environment is deemed unsafe, disrespected, or non-compliant with policies.
VII. Milage Policy
Mileage fees may apply for service locations outside a 15-mile radius. A $0.70 cent charge applies per mile beyond 15 miles.
